Updated Office Manual & Appeal Request Form

The updated Dentist Office Manual is now available to be downloaded. The new manual is effective January 1, 2013. Delta Dental has also created a new interactive appeals form that can be downloaded from the forms section of the Dentist Connection. A username and password is required to view these forms.
