Delta Dental’s Group Voluntary PPO Plans are available at no cost to the employer – employees pay the full premium and the employer administers the benefit. Plans are based on our PPO Network (see below). Employees choose the best plan to meet personal or family needs. Employees can choose from three plan options:

• Comprehensive - Covers all benefits provided in both the Preventive and Catastrophic options.
• Preventive - Covers checkups, cleanings and fillings.
• Catastrophic - Covers major dental treatment such as root canals, gum disease, dentures, bridges and crowns.

Delta Dental PPO Plan - Two Networks, Better Value
The Delta Dental Voluntary Plan is based on the Delta Dental PPO network.  In most cases, the enrollee will receive a richer benefit and have lower out-of-pocket expenses when going to a Delta Dental PPO dentist.  The enrollee has the flexibility to choose a larger network of dentists by going to a Delta Dental Premier or a non-participating dentist. 

More than 30 percent of the dentists in Iowa participate in the Delta Dental PPO network, while more than 90 percent of the dentists participate in the Premier network.  For a complete list of participating Delta Dental dentists, click here. 

Participation Requirements

• A minimum of 2 employees must enroll

• Premium Remittance
• Enrollment changes (adding and deleting subscribers)
