Welcome!

Welcome new hawk-i Members! As of July 1st, Delta Dental of Iowa is the primary administrator of the hawk-i dental benefits program. Children previously covered under the Blue Dental plan were automatically transitioned to Delta Dental of Iowa on July 1st, 2009. To ensure a smooth transition for you as a hawk-i member transferring from Blue Dental, here are a few of noteworthy points:

Dental Benefits - The dental benefits are the same under Delta Dental as they were under Blue Dental. To see your current summary of covered services and benefits click here.

New ID Cards - Delta Dental of Iowa will send you a welcome packet including identification cards in early July. The information should arrive no later than July 15th. The hawk-I ID number assigned by the State of Iowa will be the ID number Delta Dental utilizes.

Participating Dentists and Covered Services - In order to receive covered services, you must visit a participating dentist in the Delta Dental network. To determine if your provider is participating in the Delta Dental network, click here.
Dentist Appointments - If you have a dentist appointment after July 1st and have not received your new ID card from Delta Dental, make sure you are visiting a dentist participating in the Delta Dental network and tell the dentist you are a hawk-i member covered by Delta Dental.
