1. Does Delta Dental of Iowa offer a toll-free number?Delta Dental of Iowa's nationwide toll free number is 1-800-544-0718. 2. How do I confirm that my dentist is in one of the Delta Dental networks (Delta Dental Premier or Delta Dental PPO)?Information about your dentist's status with Delta Dental can be found in our online Dentist Search or by calling Delta Dental's toll free number, 1-800-544-0718. A call to your dentist can also confirm his or her participation in either of these networks. 3. Does Delta Dental require claim forms? Where should claims be sent?Most dental offices (and all Participating Dentists' offices) have standardized forms. No special claim forms are needed. Participating Delta Dental dentists will file claims for you. Although unlikely, a non-participating dentist could ask you to file your own claim. All claim processing is done right here in Iowa and should be sent directly to: Delta Dental of Iowa 4. How will I know when my claim is processed? Who can I call or e-mail if I have a question about my claim?If you have payment liability for any part of a Delta Dental claim, you will receive an Explanation of Benefits (EOB) that describes the services your dentist provided, the benefits that your plan covers, the payment made by Delta Dental, and the amount you owe to the dentist. If your services are paid at 100% by Delta Dental you will not receive an EOB. 6. What is a pre-determination?A pre-determination is a plan of treatment, completed by your dentist for services that will be provided at a future date. You and your dentist will receive a reply as to whether the treatment plan is covered under your benefits. You will also receive an estimated dollar amount for the planned procedures. A pre-determination is recommended when dental services are expected to cost more than $200. A pre-determination is valid for one year and is subject to eligibility, benefit maximums, and dentist participation at the time the services are provided. 7. What is my ID number? In most cases your ID number is your social security number and is not shown on your ID card due to security reasons. Some employers have requested an alternate number be used for your ID number and this number will be shown on your ID card. 8. Do my check ups and teeth cleaning services apply toward my annual maximum? In most cases, yes, these services do apply toward your annual maximum although we do have a few contracts where these services do not apply to the annual maximum. 9. Does Delta Dental offer Individual plans?Yes, Delta Dental offers individual plans for people who reside in the state of Iowa. Contract your Human Resources/Benefits Department and inform them of your new address. They will forward this information to Delta Dental. 11.
