Most dental offices (and all Participating Dentists' offices) have standardized forms.  No special claim forms are needed. Participating Delta Dental dentists will file claims for you.  Although unlikely, a non-participating dentist could ask you to file your own claim.  All claim processing is done right here in Iowa and should be sent directly to:

If you have payment liability for any part of a Delta Dental claim, you will receive an Explanation of Benefits (EOB) that describes the services your dentist provided, the benefits that your plan covers, the payment made by Delta Dental, and the amount you owe to the dentist.
