Delta Dental Provider Satisfaction Survey Prize Official Rules

  1. ELIGIBILTY: No purchase necessary. A purchase will not increase the chances of winning. Must be a Delta Dental of Iowa Participating Dentist and/or the office manager of a Delta Dental of Iowa Participating Dentist. The Participating Dentist's office must be located in the state of Iowa. Employees of Delta Dental of Iowa and their respective corporate affiliates, successors and/or assigns, subsidiaries and agents, advertising and promotion agencies involved in any way with this promotion and members of their immediate family (spouses, siblings, parents, children, grandparents, and grandchildren, whether as "in-law," by current or past marriage(s), remarriage(s), adoption, co-habitation or other family extension) and any other persons living in the same household of such individuals whether or not related are not eligible to participate to win a prize. Void outside of Iowa and where prohibited.

  2. HOW TO ENTER: To enter the Delta Dental Provider Satisfaction Survey prize promotion, complete the official registration form found at the end of Delta Dental's survey or mail your name, company address, phone, and email to Delta Dental of Iowa, Attn: Provider Survey, 9000 Northpark Drive, Johnston, IA 50131 after the Start Time and before the End Time. The online registration form must be completely filled out with your name, company, address, phone, and email. Limit one (1) entry per dentist or office manager. Sponsor not responsible for late, lost, damaged, illegible, incomplete, incorrect, or misdirected entries.

  3. PRIZES: The Delta Dental Provider Satisfaction Survey prize promotion will begin September 18, 2023 at 12:01 a.m. (the "Start Time") and conclude September 25, 2023 at 11:59 p.m. (the "End Time"). Winners will be selected at random by a representative of Delta Dental of Iowa on September 28, 2023. There will be a total of six (6) $50 Amazon gift card prizes for this promotion. Retail value of each prize: $50.00. Total retail value of all prizes: $300.00. Odds of winning will vary by the number of eligible entries received. Limit one (1) prize per dentist or office manager.

  4. GENERAL CONDITIONS: No cash redemption, substitution or transfer of prize permitted, except by Sponsor, who reserves the right to substitute, in whole or in part, prize of comparable or greater value if stated prize is unavailable. All federal, state and local taxes on prize and expenses relating to acceptance and use of prize are the sole responsibility of winner. Prize winners will be notified by email or phone and will be required to sign and return an affidavit of eligibility and liability/publicity release (except where prohibited by law). If a winner cannot be reached after two attempts via email or phone, prize will be forfeited and may be awarded to an alternate winner. Sponsor reserves the right, at its sole discretion, to disqualify any entrants and/or to cancel, terminate, modify or suspend this prize promotion at any time. By participating, entrants agree to: (a) be bound by these official rules and the decisions of Sponsor which shall be final and binding in all aspects relating to the promotion, (b) and consent to the use by Sponsor of entrant's phone and email address for the purpose of notifying participant if he or she is only a prize winner, and (c) release and forever discharge and covenant not to sue Delta Dental of Iowa and their affiliates, subsidiaries, distributors, agents and advertising & promotion agencies and the employees, directors and officers of each of the foregoing entities from any and all claims, demands, damages, losses, expenses (including attorney's fees) and liabilities, of any nature whatsoever, which they may now or hereafter assert, including but not limited to claims for injury, death, damages, or illness arising out of or relating to or claimed to be arising out of or relating to promotion entry, prize acceptance or use or misuse of the prize or any product or prize certificate awarded including, without limitation, personal injury, death and property damage. Acceptance of a prize constitutes winner's agreement and permission to the use of his/her name and/or photograph for advertising, publicity and promotional purposes without further notice or compensation (unless prohibited by law).  Sponsor and its agents are not responsible for any printing or other errors in promotion related material. This promotion will be governed by the laws of Iowa. Delta Dental of Iowa will not use or sell personal contact information including address, phone or email for any other purposes outside of the Provider Satisfaction Survey prize notification.
