2019 Plan Changes

In 2019, Delta Dental will no longer offer certain Small Group plans. Please see below for more details on the changes:   

• List of plans we will no longer offer – please review the full list of plans that will no longer be offered, and the plans your clients will be moved to. 
• Plan comparison charts – these charts show how your clients' current 2018 plan compares to the plan your client will be moved to in 2019. 

New Rate Structure Option

Also, in 2019, groups will have a new rate structure option. New and renewing groups will have the option to select four-tier or per-person rates. If you have a group that is renewing, and wants to switch their rate structure, please compete this form.

• Employer Choice Member Enrollment Form - member application for coverage for Small Group Employer Choice and pre-ACA tiered plans.
• 2019 Employee Choice Plans Brochure - employees choose from six standard plans.  
• Employee Choice Enrollment Form - member application for Small Group Employee Choice plans (Preventive, Preferred, Platinum Plus/Prime plans).
