Delta Dental DWP, DWP Kids and Hawki
Welcome to Delta Dental of Iowa! We are one of the dental carriers for the Dental Wellness Plan (DWP) and DWP Kids, and the only carrier for the Hawki benefit. We are happy to serve you!

How will I know who my dental carrier is?
Iowa Health and Human Services (HHS) will send Open Choice letters to all members starting in late April 2024. The letter will list which carrier you currently have. If you are a DWP or DWP Kids member and you would like to change carriers, you can do so until June 18, 2024. Your dental carrier will not be changed until July 1, 2024.
If you do not get your letter in the mail by the end of May, please call Iowa Medicaid Member Services at 1-800-338-8366 or locally in the Des Moines area at 515-256-4606.
You do not need to do anything if you are happy with your dental carrier.
How do I change my dental carrier if I am a DWP or DWP Kids member?
The letter you received included a dental carrier change form. You need to notify Iowa Medicaid that you would like to change carriers by one of the following ways:

- Mail in your Dental Carrier Choice Form: Member Services, PO Box 36510, Des Moines, IA 50315. (Mail is not processed daily and your dental carrier choice may be delayed.)

If I choose Delta Dental, when will I receive my ID card?
New Delta Dental members will receive your ID card and other information about DWP, DWP Kids or Hawki in the mail from us at the beginning of July. If you are a current member and need a new card, contact us at 1-888-472-2793.
