Value-Added Services

Dental Wellness Plan Value-Added Services

Dental Wellness Plan members may be eligible for a $10 Amazon gift card. Members can get a gift card when they complete the following activities:

  • Go to your Dental Wellness Plan dentist for a cleaning, scaling and root planing, or periodontal maintenance.
  • Answer all the questions on the PreViser Oral Health Survey.
  • Have an active Member Connection account. Learn more.

Delta Dental of Iowa will email your Amazon gift card to the email provided from your PreViser Oral Health Survey. You will get an email with an online code you can then redeem. If you have questions about when you will get your gift card or how to use your code, please call Delta Dental of Iowa’s member services at 888-472-2793. Members must also meet the following requirements to get the Amazon gift card:

  • You must have a cleaning and finish all the questions on the Oral Health Survey July 1, 2026 to June 30, 2027. 
  • You must be 19 years or older.
  • Your dental service must be with a Dental Wellness Plan in-network dentist.
  • Your dentist must send the claim to Delta Dental of Iowa within 180 days of the cleaning.
  • You must fill out all the questions on your PreViser Oral Health Survey with Delta Dental of Iowa. This includes filling out your phone number and email. Delta Dental of Iowa may use this information to contact you for outreach and to help you find care. You can complete your survey online. 
  • You must have an active Member Connection account with Delta Dental of Iowa. You can learn more about how to make an account.
  • Members can only earn an Amazon gift card for completing their dental service and PreViser Oral Health Survey once per lifetime as a DWP member.

DWP Kids & Hawki Value-Add Service

Hawki and DWP Kids members may be eligible for a $10 Amazon gift card. Members can get a gift card when they complete the following activities:

  • Go to your Hawki or DWP Kids dentist for a cleaning, scaling and root planing, or periodontal maintenance.
  • Answer all the questions on the PreViser Oral Health Survey.
  • Have an active Member Connection account. Learn more.

Delta Dental of Iowa will email your Amazon gift card to the email provided on your PreViser Oral Health Survey. You will get an email with an online code you can then redeem. If you have questions about when you will get your gift card or how to use your code, please call Delta Dental of Iowa’s member services at 800-544-0718 (Hawki) or 888-472-2793 (DWP Kids). Members must also meet the following requirements to get the Amazon gift card:

  • You must have a cleaning and finish all the questions on the PreViser Oral Health Survey July 1, 2026 to June 30, 2027. 
  • The person who completes the member’s PreViser Oral Health Survey and provides an email must match the Responsible Party on file for the member.
  • Your dental service must be with an in-network dentist for your dental plan.
  • Your dentist must send the claim to Delta Dental of Iowa within 180 days of the cleaning.
  • You must fill out all the questions on your PreViser Oral Health Survey with Delta Dental of Iowa. This includes filling out the responsible party’s phone number and email. We may use this information to contact you for outreach and to help you find care. You can complete your survey online. 
  • The member must have an active Member Connection account with Delta Dental of Iowa. You can learn more about how to make an account.
  • Members can only earn an Amazon gift card for completing their dental service and PreViser Oral Health Survey once per lifetime as a Hawki or DWP Kids member.

Please note, Delta Dental of Iowa will only administer gift cards to the member’s responsible party or members who are 18 years of age or older. Delta Dental of Iowa will not administer the Value-Added Service to anyone who is not the responsible party or an adult member. This is currently a pilot and may be discontinued after December 31, 2026.
