We’re happy you’re here! To make your dental benefits easy to understand and help you get the care you need, here are some tips to get you started with your Dental Wellness Plan (DWP), DWP Kids, or Hawki plan.

1. Check the mail for your new member packet

You will get a packet in the mail with your ID card and other important information about your benefits. You should take your Delta Dental ID card with you to all your dental appointments.

2. Take your PreViser Oral Health Survey

All new members should fill out their PreViser Oral Health Survey. This is a short survey that helps us get you the resources and customized care you need. This is different than the self-assessment you take with your Managed Care Organization.

Take the survey for DWP Members.
Take the survey for your child with DWP Kids or Hawki.

3. Create your Member Connection account

Member Connection gives you easy access to helpful information. You can get a copy of your ID Card, view your claims, find helpful member resources, and more. Learn more about how to make your Member Connection account.

4. Learn more about your benefits

We want to make sure you have all the resources you need! You can learn more about your plan, read your member handbook, and find other important documents on our resources pages.

5. Make an appointment with your dentist

You should see your dentist every six months for a cleaning and exam. You can use our Find a Provider tool to check if your dentist is part of your plan’s network or to find a new dentist.
